To enroll your facility*, please follow these instructions:
Step #1: Please complete the online application form
Step# 2: Review and agree to ICAA Code of Conduct
Step #3: Double check completed form for errors, then submit.
Please ensure you list on your application form the levels of function your organization is best suited to serve.
*The application form review process is 5 business days. Upon acceptance, your organization will appear in the online locator.
